It is, however, labor intensive and time consuming and it is not always possible to isolate sufficient tissue for DNA extraction. The aim of this study was to determine whether a highly specific set of oligonucleotide primers would be useful for the sensitive and specific identification of P. carinii. A nested PCR assay was developed using primers from the mitochondrial large subunit ribosomal RNA gene (mtLSU). This method yielded positive amplification in all culture-proven cases of P. carinii and in 12 of 18 autopsy specimens. Nested PCR was less sensitive in the detection of P. carinii in non-PCP and non-P. carinii cases. All amplifications obtained with nested PCR were positive for the control DNA, pUC8 plasmid DNA. The method was applied to samples from 47 consecutive autopsy cases. Thirty-four (72%) cases were P. carinii pneumonia (PCP) cases, 11 (24%) were non-PCP, and 1 case (2%) was both PCP and non-PCP.
